Employers hand out this one-page form, and its image goes in Twilio's "Opt-in method proof" box. Following counsel, it has one bilingual disclosure, one checkbox, one signature and a role field, so a single form serves employees and supervisors.

Opt-in method

Paper form

Message flow (consent box)

Employees and supervisors of WagePath's business customers opt in on a WagePath consent form (Form WP-SMS-1, English and Spanish) that the employer provides, on paper or signed electronically, at hire or when the employer starts the program. The recipient writes their mobile number, marks their role, checks one consent box, and signs and dates the form. The form names the program, WagePath Workforce Time & Compliance Alerts, lists the message types, and states: "Message frequency varies and may be up to 8 messages per day. Message and data rates may apply. Reply STOP to opt out at any time or HELP for help. Consent is voluntary and is not a condition of employment." It links to wagepath.com/terms and wagepath.com/privacy. The employer keeps the signed form and records the consent in WagePath (mobile number, date, form version, collection method, recorded by). WagePath texts only numbers with recorded consent, and immediately sends and keeps an enrollment confirmation. Mobile numbers and consent data are not shared with third parties for marketing.
